What we need from you
Power of Attorney
- Name of the person you wish to designate to have the authority to act on your behalf and a back-up person in case first person is unwilling/unable to act.
Medical Power of Attorney
- Name of the person you wish to designate to participate in your medical care if needed or desired and a back-up person in case first person is unwilling/ unable to act.
Living Will
- Name of the person you wish to designate as your agent and a back-up person in case first person is unwilling/unable to act.
- Decide whether you wish artificial life support and/or organ donation.
Will
- Name of the person you wish to designate as your executor and a back-up person in case first person is unwilling/unable to act.
- Who you wish leave your estate to, in what shares, etc.
Parents with minor children (under 18)
- Name of guardian for the children – who will raise them
- Name of person to be handle the (estate) money you leave for your children
- REMEMBER, IF YOUR CHILDREN ARE OVER 18,
YOU ARE NO LONGER THEIR LEGAL GUARDIAN –
CHILDREN IN COLLEGE NEED THESE DOCUMENTS!!!
